Here are just a few of our lawsuit wins for our clients around the United States.
Medical Service Associate - Baltimore, MD
Sarcoidosis, Depression, Anxiety
“A Medical Service Associate from Maryland developed Sarcoidosis and significant depression and anxiety resulting from the cancer treatment. She left work during her chemotherapy and received long-term disability benefits, but when the chemo stopped, Sedgwick stopped paying her benefits.
Worried about her lost income and struggling to explain her ongoing issues to Sedgwick, she contacted Fields Disability for help with her appeal. Our attorneys drafted an appeal based on our client’s most recent medical treatment. Sedgwick reversed its denial decision, and our client received a lump-sum payment for back pay and ongoing monthly benefits.”
Mechanic - North Vernon, IN
Systemic Degenerative Bone Disease, Sciatic Nerve Damage, Anxiety, Depression, Insomnia, IBS
“A mechanic from Indiana developed systemic degenerative bone disease and was forced out of work. After leaving work, she began suffering from extensive mental health issues. She applied for and was granted Long Term Disability benefits. She remained on claim for two years before Unum canceled her benefits when her policy changed the definition of “Totally Disabled.”
Knowing she needed help appealing Unum’s decision, she retained Fields Disability to fight for her case. The Fields team began requesting updated medical records from her providers and crafting an appeal of the insurer’s arguments in their denial letter. The Fields attorneys submitted their appeal and within a few weeks, Unum had overturned their prior denial and were issuing a check for back benefits.”
County Recording Clerk - Maplewood, MN
Rheumatoid Arthritis, Trigger Finger, Chronic Low Back Pain, Chronic Right Wrist Pain and Swelling, Osteoarthritis Bilaterally in the Knees, Chronic Left Hip Pain, Chronic Fatigue
“A County Recording Clerk in Minnesota ceased working at the recommendation of her doctors due to a progression of her rheumatoid arthritis, trigger finger, chronic fatigue, and osteoarthritis in her knees, hips, and low back. She applied for disability benefits, but Madison National denied her Short Term Disability benefits and her application for Long Term Disability benefits. Unsure where to turn next, she reached out to the Fields Disability to appeal her Short Term and Long Term claim decisions.
The Fields Disability team gathered additional medical records, and a narrative report from our client’s treating doctors. The Fields attorneys submitted an appeal, but Madison National reviewed and upheld its original decision because our client returned to work after surgery and physical therapy, and Madison National used the return to work to assert that she was never disabled from performing her own occupation.
With a second appeal available, our team obtained additional updated medical evidence and additional supportive narrative reports stating that our client’s ability to return to work was due to her physical therapy and time off work to recover from her surgery. After review, Madison National overturned its denial and approved all of her Short Term Disability benefits and two months of Long Term Disability benefits.”
Police Officer - Lakeville, MN
PTSD, Alcohol Use Disorder, Degenerative Left Hip Disorder, Multi-level Degenerative Disc Disease, Disc Bulges, Nerve Root Compression
“A Police Officer in Minnesota was in an officer involved shooting and developed severe PTSD. That condition, alongside an ongoing degenerative disc disorder, ultimately forced him out of work. He applied for Long Term Disability benefits, but Madison National denied his claim for benefits, stating that his conditions didn’t prevent him from doing his job.
Knowing he needed help, he retained Fields Disability to fight for his benefits. Our team requested additional updated documentation, and supporting reports from his treating providers. The managing attorney wrote an appeal critical of the reasoning for their denial with additional supporting documentation. After months of reviewing the appeal, the insurer overturned the denial. Our client was paid for all past due benefits, and was reapproved for ongoing benefits.”
Mental Health Practitioner - Eyota, MN
Functional Neurological Disorder
“A Mental Health Practitioner worked at her local school district in Minnesota for many years until she developed a neurological disorder that caused dizziness, severe fatigue, inability to concentrate, and difficulty with speech that forced her out of work. She filed for disability benefits under her insurance policy, but Madison National denied her claim and stated that the documents did not support the inability to work in her occupation.
Unsure of what to do next, she turned to Fields Disability for help. Our team requested medical records from all her treating providers including a supportive statement from her primary care physician, confirming her new diagnosis of functional neurological disorder, the objective findings, and the provider’s opinion on her ability to work. Fields attorneys filed all of the additional information on appeal. After reviewing all the evidence and a physical examination, her benefits were approved, and she received payment for the past due benefits.”
