Finance and Administration Manager - Appleton, MN
Scheuermann’s Disease, Bilateral Carpal Tunnel Syndrome
“An Administrative Manager for a large railroad company in Minnesota developed lower back and neck conditions that made it very difficult for her to perform her normal work duties. These conditions, combined with bilateral hand pain from carpal tunnel syndrome, made it impossible for her to perform the computer work demanded of her on a full-time work schedule. After working with these conditions, she left work at the at the advice of her doctors. She filed a claim for disability benefits with The Hartford and was approved. A few months later, her insurer terminated her claim, offering litle explanation. Confused by the decision, she searched for help online and found Fields Disability.
Our team wrote requests for reports to our client’s treating providers. To assess our client’s functional abilities, the Fields Disability attorneys referred her to a Functional Capacity Evaluation (FCE). The FCE revealed that our client might be able to perform some modified form of desk work. Our attorneys submitted the report to a vocational expert for review and comment, and, after review, the vocational expert concluded that no gainful work in the economy existed with our client’s restrictions and limitations. The Fields Disability attorneys submitted the additional information on appeal. The Hartford paid our client all past due benefits, and reinstated benefits.”

Migraines, Convergence Disorder, Light Sensitivity, Sound Sensitivity, Visual Issues
“A Pediatric Physical Therapist from Minnesota suffered from severe and sudden migraines. Her condition caused her to have extreme sensitivity to light and sound, and made concentrating at work impossible. She applied for Long Term Disability through her employer’s insurance policy. After paying benefits for about four months, Prudential denied her benefits stating her records did not support restrictions and limitations preventing her from returning to work.
Worried about her lost income and unable to return to work, she contacted Fields Disability for help. Our disabilty team obtained the claim file from the insurance company, and additional medical documentation with expert reports. When our attorneys reviewed the file, it was obvious that the insurer overlooked key medical records. Fields Disability attorneys submitted the updated documentation, and a strongly supported appeal letter. After reviewing the updated information, the insurance company paid our client back benefits and ongoing monthly benefits.”

Bipolar II
“A Customer Service Representative in North Carolina stopped working due to emotional instability from his bipolar disorder. He applied for Long Term Disability benefits, but Metlife denied his claim stating that his medical records failed to show he was unable to return to work.
After learning this unfortunate news, he recognized that he needed help. He found Fields Disability and reached out for assistance. After reviewing his case, the Fields Disability attorneys determined his benefits were wrongly denied and started to work on an appeal researching relevant legal arguments to present on appeal. She started receiving benefits, but after a period of time, Liberty Mutual terminated her Long Term Disability claim, stating that she did not meet the definition of disabled in the policy.

Utilizing the additional documentation, the Fields Disability attorneys submitted a comprehensive appeal. After receiving the appeal, Liberty Mutual restored our client’s benefits.”
Music Teacher - Dayton, NJ
Lupus, Diabetes, Chronic Migraines, Neuropathy
“A Music Teacher from New Jersey was taken out of work by her physicians due to Lupus, chronic migraines, and neuropathy. These conditions impaired her balance requiring her to use a cane to walk, impaired cognitive ability, and caused chronic pain and fatigue. She applied for disability benefits, but Prudential denied her benefits stating that her medical records did not provide substantial evidence to prove that she couldn’t do her job.
She decided to contact Fields Disability for assistance. Our disability team started gathering the necessary documentation to file the appeal. Our attorneys drafted the first appeal using all medical records and reports who supported our client’s claim for disability. Prudential upheld its denial stating that she had not satisfied the elimination period. The Fields Disability attorneys drafted another appeal that documented disabling medical evidence during the period of time that Prudential deemed our client not disabled. After the second appeal, Prudential paid our client back benefits and was put on claim.”
